I looked in the same spot on my '99 and it had already been replaced by the previous owner with a piece of fuel hose...so I guess that's an option. Works fine.
I have see some elbows like that in the "Help" packages at Autozone, i bought a package that had 4 or 5 different angles in it, as i was needing one for my avalanche that looked about like that one in your picture, and was told it was a dealer item also. you may want to see if it has what you are looking for.

3/8 fuel hose or vac hose will work fine. Need to fix it or if you crank on the engine you will puke oil out everywhere. That's the PVC crossover from the driver's side PVC nipple.
Be careful when you take the old off even though it is cracked. The crossover line is plastic and they can get brittle with heat. then you are begging a new plastic line and it is tough to route in the right place.
If you want to fix it for good, an LS6 2004 valley cover GM # 12577927, a 12568011
plug, the hose pigtail, GM 12594779 and a couple feet of 3/8" vacuum hose, and you can rid your engine of the entire PVC plumbing you now have. With this valley cover, the PVC valve is inside the valley cover. The new valley cover comes with the GM gasket.
One complication is that you will need to cut a little piece out of your block for this mod, but if you mask off the engine and use a dremel tool with cutoff wheels, it is a 10 minute job. JimX' s LS site has good instructions for this.
I've done this mod to two of my C5's and never looked back. Also makes install of a catch-can simpler and maybe more effective.
My 98 had on-going failure of all the rubber fittings in this sytem, and eventually the plastic hoses started to develop cracks too. I got some brass fittings to re-create the "Y" junction behind the intake manifold and replaced ALL the hoses and angles with new fiber reinforced fuel hose. I also used some size adapters to convert to an in-line PVC valve from an eariler era GM engine, that is far less expensive than the C5 specific valve. Unless you are going for a Concours factory correct install to win at a car show, do not be afraid to use superior materials and some intellegent DIY solutions to fix these hose issues.
