Are you getting any codes? Any blinking light to indicate a misfire?
You are describing the symptoms of a possible leaking head gasket. Overnight coolant leaks into one of the cylinders and upon starting it takes a few seconds to clear the cylinder that has the coolant in it and pick it up so it is firing again.
If the shudder doesn't happen until after you have put it into Drive...then the above may not be your problem, but it does sound like you are dropping a cylinder...maybe something as simple as time for a set of plugs or a bad plug wire.
If that is in fact the problem it can often be diagnosed in several ways...since you aren't going to do the work, guess there is no point in covering that.

Well, that's good that you have the extended warranty...should give some peace of mind. If you haven't had an overheating problem recently, then it probably isn't a head gasket.
Sometimes a shudder when the tranny shifts into overdrive, turns out to be in reality a misfire and not a transmission problem. But it doesn't sound like this is what you are describing.
Plug or ignition wire is my guess. Let us know what they find.
