Help!!! AC / heat fan problems

Hi everyone
I am new to the forum and am having an issue with my ac / heat fan. The fan only works on 1 and 5. I replaced the fan because it went bad making this crazy loud noise. Now there is this problem. Any suggestions?

It's the manual control. Sorry I didn't specify. Do you have a picture or part number that would go with that?
