Can Anyone Identify this CAI Kit?
Most of those names you dropped have probably taken classes from Banish. He's done numerous workshops teaching Diablosport engineers, independent shop tuners, as well as what you would refer to as "goofball" hobbyist tuners.
The tune has a MAF calibration from a C6 that I assume is supposed to match the Granatelli MAF sensor, and the cylinder volume constant has been adjusted to match an overbore on the block. The only other bits of "tuning" that were done were adding ridiculous amounts of spark advance in cells (load ranges) the engine will never operate in. And even though it's apparently had at least two "tuners" that have touched it with one obviously a pro (I won't mention his shop's name but it's in Illinois), none of them were considerate enough to uncheck the CAGS option so the skip shift solenoid dummy resistor could be eliminated.
I've yet to do any logging to see how far off the MAF calibration is, but I'd bet you all my marbles I could improve upon it with minimal effort. Certainly the VE table, since it's bone stock and the car has CNC ported 241 heads and an LS6 intake (both of which were also unknown to the previous owner) along with longtube headers.

I'm looking for a stock airbox / bridge / TB coupler that will connect to this MAF. It appears to be an OE 85mm that's been somehow violated by Granatelli as evidenced by their sticker over the cover plate. I see several on vettenuts - one of which says it's a Z-06 and possibly includes the factory MAF though I will need to confirm that. Does anyone know if the Z-06 intake is the same as an LS1 C5?
