Certified odometer statement
I made the mistake of purchasing a dealer extended warranty. Thought I was getting the GMPP. What I got was an AMPP. Finance guy waved the GN brochure in our faces and asked if we wanted it. He filled out the paper work and we signed. I can cancel but must send in Certified odometer statement. Anyone know what and who can give me one.

You might want to ask the warranty holder what is acceptable. I did a quick search and found a lot but nothing that said exactly what you are looking for. Maybe they will accept a common form like this one
