Here's a template developed by someone else. I re-worked it in visio to the exact dimensions called for. This is a .png of the 3-page visio file. It won't fit here, but the file on the share site is to scale. I have this up on a file-share site PM me for the link (but be patient - I am on travel)

this .png is 2987×772 pixels at full resolution



With this template you can make your own out of whatever material and finish you want.
