I actually removed the wipers and replaced both wipers and blades the first time I changed them out. After that, the passenger side wiper would make a gosh awful humming noise. The next time I changed the blades out, I put the 'wing' back on. No more humming.
Obviously the wingy-thingy is useful above some certain speed, we just don't know what that speed is.
I've been very pleased with the original GM blades, so I buy refills one year (your local dealer may be selling aftermarket, so ask for GM) and replace the whole assembly the next year since some of the plastic parts get brittle.
I think I paid something like $20 for both driver and passenger assemblies at my local dealer. Very hard to beat that pricing.
Paid $11 something a piece at the dealer. The originals lasted 3.5 years with about 50k miles on them and had seen plenty of rain. I wouldn't bother with refills for that kind of price/performance ratio from the stock wipers.
