Happened on my Ford Taurus SHO. It was the fan relay switch. Eventually lead to a recall. Not sure if that's your problem.

On my 2002, I sucked up a plastic bag and it wound itself around a fan motor. There are 2, and they kick it at different times. I would check to make sure that both fans are turning on. It sounds like you only have 1.

What kind of outside temps are you driving in? Here in L.A. it has been hot and humid. When I get stuck in traffic (very slow stop and go) where my outside temp gauge reads in the high 90's or 100's, my car always runs hot with the AC on. There's no air moving over the ac condenser to cool it off and that heats up the air being pulled to my radiator. Once traffic clears up and I drive for a bit the temp comes down. I added a 8" fan to my condenser and put in a manual switch so I can cool the condenser when sitting still in a traffic jam and it took care of the problem.

If you have access to a scan tool like a Tech 2 you can go to the special functions section and manually activate the fan. Check for power at the connector at the fan when the engine is running and ac is on. If there is no power then back track from there. Yes, the PCM turns the fan on and off as commanded.
