Mine started to slip a little bit after about 2 years and about 9000 miles of 100% street driving and would hop a rib on the pulley if i got in and out of the throttle really abruptly (never kicked off though).
I swapped it out and it had VISIBLY stretched after that time. Every 18 months or so seems like a reasonable interval if you're not pounding your car at the track I think.

Jumping ribs sounds like an alignment issue.
That happened to me when I had a belt out of alignment. The rib that hopped off the pulley also came apart and damaged the hoodliner.

Shawn decided to see how many passes he can make on one six rib belt, I think it lasted somewhere between the 4-500 mark. Which is just silly. The car now is nearing 1300 passes on the original stock engine, and the belt has been changed three times total.

It only started doing it when the belt was at the end of its life....it had stretched to the point where the spring tensioner was nearly bottomed out and couldn't keep the proper tension on it anymore.
It's been rock solid since I put the new belt on it.
