Default Destroyed another belt

So I was going out today for some spirited driving since I never touched her this weekend and I was getting on her pretty good and just as I hit redline I felt a little shake like I hit something in the road. Well next thing I know the power steering was gone... I was confused for a second but then I remembered I just replaced the serpentine belt and I probably just completely lost the new one. Suspicion was confirmed when I started overheating. I pulled over as fast as I could and she puked up a little coolant and threw the engine light for overheating.

I was surprised how cold some of the parts where. The supercharger itself was almost ambient temp. The only hot part was a coolant hose. I got my new gatorback belt on and started her up. The bolt on the back of my tensioner was loose again. I'm not sure why it keeps coming loose but every time I dropped a belt it has been loose. Anyone with a Procharger know the proper way to install the belt and tension it? I don't want it to start slipping and come off again I barely had time to get off the road.

I cleared the engine light and also the change oil recommendation. I checked out the oil and it was clean and full so I didn't lose any. Didn't look like the heat broke it down any either... hopefully the coolant overheating just freaked the car out a little. I'm mad at myself for letting some fun get in the way of keeping my car in working order but these belts shouldn't be getting tossed off.

Hopefully the gatorback will hold up but I'm getting a new one just in case. Also I'm staying out of boost until I get everything figured out. But I guess the good part is the damn reduce engine power mode didn't come on.....

Can you put a lock washer on that bolt to keep it from vibrating loose?

I'm not sure if I should put one on there or not. The guy who did the install sent me an email with the install instructions and I guess my belt is slightly loose. There are 3 notches on the procharger tensioner. The one in the back needs to be lined up with the 2nd notch on the front. I've been putting it right between the two. I'm going to adjust her and start her up right now and see how it sounds. It also said after about 100 miles tighten the belt again. Hopefully I can get the tension right and tighten that rear bolt down enough.

I'll probably just short shift the car around 6k rpm instead of redline.
