My middle headlight recently burned out. I purchased a new one for $100.00 (yiiiiiiiikes) and according the manual, this middle headlight that burned out, requires the car to be taken to the dealer to be replaced. 1) does anyone have any idea of the porposed cost for this?
2) are there any other options?

The center bulb, D1S or HID buld generally run around 60 to 80 bucks. Many have changed their own bulb. Do a search here using "HID BULB" and read the many post concerning changing the bulb and how to with pics. BTW the dealer may have only charged a total of 100.00 including the bulb.

If you're talking about the HID low beam bulbs, the owner's manual says that because of safety. It is possible to get a serious shock from the electrics. I haven't seen any DIY change out threads with pics but there are several with word descriptions. Here's one where I linked 4 other threads. Read all of them so you have the best understanding of what's involved. Heed the advice to be careful not to drop the screws, use a magnetized screw driver.
