Unfortunately, GM does not sell only the motor, but the fan assembly including the plastic grill Part # 15819952 for the whole assembly
$260.00 or so at forum vendors (GM partsdirect or at CultragPerformance)
Try to find an electrical supplier in your area that has replacement motors.
I found one in my small city, so I'm sure you have one in your area. They can match any motor from any application. You just need the info stamped on the side of that motor that's bad.
I've got one. I'll get some pictures today. Not sure of your location, but it looks like you're overseas. Would like to get $60 plus shipping. PM me if your interested.

Good Luck
Thanks for all these info, is it hard to release the old motor?
I've never heard of the motor dying, but there have been many reports, including mine twice, where the connector shown in erick_e's 3rd photo has severely burned contacts. If the contacts burn out, the fan quits. I hope you checked this first. I did a hardwire bypass after the second one burned up.

I tested the motor, the fan not going for the fastest speed + the fan blades were very heavy and hard to move. So the problem is from the motor, evry thing else work great!
Ok can you tell me what size of socket I to use for this bolt?
To get it out?!
Once you get the shroud assembly off, hold the fan blade still and rotate the 15mm nut/shaft counterclockwise to unlock the three locking tabs and the fan blade will come off. Then you can remove the bolts holding the motor to the shroud to remove it.
Once you have the motor in hand, see if you can spin it or not by hand.
Have not pulled a vet fan motor down to service it, but even sealed motors with bent over tabs can be pulled apart to either clean the Bronze bushing,/clean and re-grease or replace the bearings.
Worst case, you pick up erick_e for around $100 with shipping, and Bob's your uncle.
