bought mine from Dennis and it is not in the My chevrolet website, called and asked him about it and he said some do not get loaded into the system.. if that happens you have to call the number on the printed out copy... My service guy at the dealership has a copy and I carry one with me...
On a good note, I have used it 2 times and they have accepted both and paid them on the spot, a new 600 watt fan is getting installed soon

mine has the "fan go to sleep mod from chevy" meaning if you drive long distance over 50 miles or so, I did 62, the fan will not start back up when you stop or slow down... it then overheats until you stop and start the car...
