I put both feet out first and then I use my left elbow and forearm on the flat of the quarter panel to help push myself up. You kind of roll onto your forearm and out of the car. Easy Peasy!
I have severe arthritis in my right knee, so maybe this will work for you too.
1. Left leg followed by right leg out to pavement
2. Facing out, butt still in seat, feet flat on pavement, put your right elbow on the A pillar
3. Stand up using legs and right arm pressure at the same time
