I have had a friend request pending from Black Ice C4 in my in box for months. I have accepted the request but it stays as a request, doesn't get added as a friend.
Can someone clear this up for me?

You must select the box in the corner of the friend's avatar to actually accept it. If you don't, and just submit the form (Save Changes), the friend(s) won't be added by the system will not see any error.

I did enter the member's name in the text box and that worked.
