You need to first place a check mark in the member's avatar box before you can apply (allow or disallow) the request. The following example is of the check box that needs to be selected when accepting or denying Friendship requests:
When the page is loaded, those who do not have a Friendship with you, but are *pending*, will not have the check box automatically selected. If you simply hit the save changes, you won't receive any error. This seems to cause people to believe they accepted the Friendship requests.
