where is the insert button?

where is the insert button?
After clicking on the "insert image" button I get the page and I type in the url and below that on the right is supposed to be an "insert button"
The page is cut off below where you type in the url so the insert button cannot even be seen...how can that be fixed?

What screen resolution and browser and version are you using?

Windows Explorer 8...not sure about the resolution...where can I find out...anyway I have always been able to insert a URL and had it work until just recently. Format changed to post pics?
I see that my resolution is 1440x900....is that the problem? It says that is what is recommended...
orientation is set to landscape...should it be portrait?
WTF? you must have fixed it because it is now working...I can see the "insert" button...
