Both.... If the 3 levers that attach to the side of the trans towards the front loosen it can throw off the alignment. If the bolts that attach the shifter to the car loosen it can throw off the alignment also. Chances are if this has happened recently you have some loose bolts.

that it is probably a linkage adjustment problem, but it might also be as simple as putting some lubricant on the reverse lock out mechanism at the base of the shifter mechanism.
