Need some info please

Im in the process of repainting my 74 coupe and the blue sticker on the drivers side door is very deteriorated and I need a new one. Unfortunantly I am missing some info on it. All I can make out is the following:

GVW_________040 GAWR FRT 1922 REAR 2118

1Z37J45400795
PASSENGER CAR

hope someone can help me fill in the missing blanks.
