Are you sure the ones you've seen that you thought were cut out weren't just correctly painted black to look like the others that are cut out?
I find it more common that like myself owners haven't blacked out the fourth vent and instead left it body color.
The 4th louver was never cut out. The area behind it, does not open to the engine compartment. It actually seals the plenum area of the cowl, where fresh air is fed to the interior.
You don't want to open it up. As DZRick said, paint it flat black, as it originally was.
Glad I read this. I just put a new one piece front end on my 69. All Louvers are cut out on the fenders including the rear slots. I was just going to leave them open but now sounds like I need to block them.

Check the AIM. There are bonding panels behind each of the rear vents which close them off. These panels are needed to mount the fenders at the cowl.
