I found the pictured tube had fallen off, in front of radiator. Seems it goes from a T connector in the center to the passenger side headlight assembly, I can feel an empty nipple on rear side of actuator.
In the second picture, it is the 3rd tube from the top, attaching to a T connector, not the pink or yellow striped tubes, just black (finger on it).
Can someone please help me identify these 3 tubes (pink, yellow, black), I assume 1 is the vacuum line to the lights (looks to be the yellow based on what I can trace back) and 1 maybe is the headlight washer or 2nd vacuum tube (I think actuator has 2 vacuum nipples but not sure)?
Where is the reservoir for the headlight washers anyway?

What year is your car?
Perhaps these photos will help.
This is a 71 with 2 relays.
Small 'control' hose to pipe on relay vacuum can.
Large yellow vacuum supply hose to middle pipe on relay.
Large green vacuum hose... lower pipe on relay to pipe on forward side of actuator .
Large red vacuum hose... upper pipe on relay to rear side of actuator.
Headlamp washers were done away with early in the 71 model year so aren't in the photos.
Perhaps trace the hoses from the nozzles on the back side of the headlight bezels to the T's at the washer hoses running to the headlight area.
If you don't have an Assembly Instruction Manual for your car you should buy one!

69 (have tried to put that in my signature but does not retain).
I believe I am referring to the lowermost of the tubes: "Large green vacuum hose... lower pipe on relay to pipe on forward side of actuator."
Since the tube is so loose as to fall off, I imagine I should replace since likely leaking vacuum.
I have the AIM, could not locate correct page!

69 cars typically had one relay…. so a short hose came off the relay and then split at a T. Each green striped hose went to the front side of the actuator.
Green is 'go'… open.
Red is 'stop'…close.
Is the pipe on the relay broken or is the hose loose on the pipe?

Am I supposed to have 2 relays, one for each actuator, on my 69 like your 71?
Seems I have 1 relay, with "T" fittings splitting off to both actuators.
Like Alan said, your 69 will have one rely attached to the header bar by the nose. There is a second relay but it will be located either under the passenger front fender on cars up until around March (I think) and then it was relocated to the firewall next to the master cylinder or brake booster if you have PB.
Hose ends just loose on both the relay "T" pipe and the actuator. Probably just stretched out and need a new one unless there is an easy trick to tighten the fittings.

cut about half inch off each end then they should push on firm if not wrong size hose as early c3 light canisters had smaller hose fittings than 70's and up C3s
