Default Throw Out Bearing Alignment

I am in the process of completing a conversion to a 4 speed on my 1979 C3, and discovered that after the clutch fork is installed on the ball stud and the throw out bearing installed on the clutch fork (all correctly), the center of the throw out bearing is a bit off center from the center of crankshaft and pilot bearing hole. When I install the 4 speed transmission it aligns the throw bearing, but in so doing, the bearing gets pulled out of the fork fingers about 1/2" - 3/4". Has anyone encountered a similar problem when doing a conversion? Everything fits together when assembled, just not sure whether it is correct to have the throw out bearing not fully seated in the clutch fork fingers. Any help or suggestions would be much appreciated.

The clutch fork does not fully seat on the throw out bearing. The center of the two stamped in slightly curved "pads" on the fork end should be roughly perpendicular to the center line of the shaft depending if it's fully engaged, centered, or at full release but it does not slide all the way onto the bearing. It's sort of "self centering" off the pivot stud.
