I halfway remember buying mine at Corvette America but they're gone now. I had to look carefully through the options to find a blank plate. This was seven years ago, so ....
I checked into engraving and decided I could make something suitable out of transparent stick ons from my printer. Wanted to get the font and size right. If you go to an engraver, I recommend you first figure that detail before they get the job.

Paragon has all the years and different option plates. Send them a message and ask if they can print one for you.
Not so, it can be anything you want, just like putting the right decal on your air cleaner.
numbers matching. number on hood matches sticker on air filter actually getting it printed would be easier. you will spend 50 to 100 bucks on stamps and they won't line up.

The 1969 plate has 4 spaces for info, not 3 like the above plate. I have not seen the original blanks available. I just bought an original type with numbers and had a local engraver laser engrave a blank with my numbers to apply over the stock ones. I gave him the info like Ignatz has for the pattern.
Aren't the numbers on the aftermarket plates just silk-screened ink/paint? If so, just buy the correct plate design, use lacquer thinner to remove the numbers you don't want, then have 'new' info either etched into the plate or get some transfer numbers in the correct size and font and make them whatever you want. You can shoot some clear lacquer over the new numbers to protect them.
In fact, you might be able to re-number the original plate if it's in good shape.
