Mine is 614 and it was built on April 29th, the 16th week of '76.
Was yours built in Dec. '75? Trim tag would be E with the number around 15, I'm guessing...

Jack suppliers changed in 76 from Ausco (A) to Stant Manufacturing Products (SMP) as well as the date code format. The SMP jacks used the three digits as your jack shows with 5 = year and 48 = week assembled. It should have an SMP on the bottom for a Nov/Dec 1975 assemble jack, The jack date code tells us your trim tag date code should be a 76 is an early build...75ish.

You can get the build date from your trim tag located below the left windshield pillar.
Look to the right of Chevrolet at the top of the tag.
If it was built in Feb there should be a letter G followed by the date it was built.

'75s didn't have the stamp on the rivet. Or the week made.
The number was the year, 5 for '75. Of sort of 5 in your case.
And the letter was the month, A for January.
When GM did it this way the letter I wasn't used, so September is J & December M...
