Correct Alternator Adjustment Bolt

I have purchased what was described as "correct" Alternator Adjustmnt Bolt (Top Bolt), for my car.
The bolt has a lock washer and flat washer however they will slip off the shaft of the bolt.
My question is does the correct bolt have the lock washer and flat washer pressed onto the bolt so it will not slip off the shaft ???

I am pretty sure mine will slip off. Been a while since it was off.
The lock washer nest to the head of the bolt.

Chris,
The washers are loose, not captive. The flat washer is thick, about .090", so it doesn't deform against the mounting bracket.
