Depends on how wiped out the lobe is. I have seen some cams with lobes that are pretty bad and it seems to run fine. Of course it was a mild cam to start with. Your power level will drop off and yes it will start to miss at a higher rpm. If you suspect anything pull the valve covers and measure the lift.
In my case (solid lifter 365 hp), a worn exhaust lobe resulted in pressure in the crankcase and some excess oil backing up into the aircleaner. This caused eventual fouling of plugs - but still ran strong.

What if you have hydraulic lifters? What would be the symptoms?
I experienced a wiped lobe with Hyd. lifters (comp cam 268XE). First symptom was excess play in the valvetrain on this particular lifter which showed itself in the form of a ticking sound. Same sound as if you had a slightly loose rocker that needed adjusting. I verified was a wiped lobe by removing valve cover, remove spark plugs, turn engine slowly with wrench on crank and watch the valve train while looking for rockers that don't move as high as others.
