There are quite a few custom header builders out there. We put alot of them on this page. http://www.headerdesign.com/extras/resources.asp . You can also ask the local racers where they get their work done.
The easiest thing to do is buy a quality set of off-the-shelf headers with nearly equal-length primaries, and the correct diameter primaries. Then have a shop cut off the collector and modify the primaries, weld on the correct collector (they come in many sizes), and collector body back to the crossover detail.
