Sometimes in order to get the car to shift properly I need to let up on the gas then re-aply it in order to get the thing to shift. It doesn't happen all the time but is starting to worry me. I've been told the Torque Converting needs replaced and my guy at Autozone says it could be something easy like a shifter selinoid.
It's a vacuum modulator problem. Either the modulator is bad [you can't find the original type of modulators anymore, but any car parts place can sell you a decent replacement part], the vacuum feed line is disconnected somewhere on its route, or the vacuum line is plugged up so that vacuum is not getting to the modulator.
Or the vacuum hose is cracked and leaking air into the line. This reduces the vacuum that the modulator sees which raises the shift point(s); letting off the throttle raises the vacuum and allows the modulator to see more vacuum and trigger the shift.
