1. Does the shifter go into the gate for reverse?
2. Did you check the reverse rod to see if the adjustment nut loosened?
3. Did you remove the reverse rod from the reverse shift lever and try to move the lever by hand?

Yes to all three. Lever will move when disconnected or at least the shifter arm does. When reconnected, reverse locks up.
