clip that goes in the back side of the handle (available at all parts stores).
You put the clip in the slots - and push it onto the splined shaft.
EDIT - you mean the rotating **** on the end. I think it's a peened-over
rivet that hold that on. You'll have to get a door handle clip tool to get the
crank off. Then, you might be able to peen the rivet back over - but it won't
last for too long (my guess). You might want to get a new or used one.

I had the rotating **** come off, I was able to put it back by removing the arem with the clip tool and screwed it back on with a self tapping screw in the remains of the peened over rivet. I then took the self tap out again applied plenty of locktite and a plain washer, screwed it back in and its been fine for about a year now. Of course having said that I will get into the car this weekend, wind the window down and....
