I am wondering if I have a clutch problem. Clutch does not slip under full power in 1st, 2nd, and 3rd. If I shift full throtle into 4th the clutch slides and shudders until hook up. Is my clutch going bad?

Damn, at WOT from 3rd into 4th at 5,600 RPMS you are doing about 97 MPH. The 4th gear would take the engine down to approx. 4,000+ RPMS.

Do you have excessive vibration on the car?

Its good to know this test took place at the track

Fourth gear is going to put the most load on your clutch for that type of failure, so yes it does sound like your clutch is wearing out.
