All the ones I've ever seen have that plaque. As far as the VIN. I thought the "5" was the start of the number sequence... or "P"... at the end of the VIN.
Another way to tell if it is an LT4 is to get a flash light and take off the oil cap. Look inside and on the head it will say LT4.
Mine don't. That started with the later cars although I'm unsure of how late.
FWIW, if you really have one torn apart, late LT4 pistons have "LT4" cast on the underside but the early ones don't. I'll have to upload a pic of mine and a late LT4 piston side by side.

I wonder how late too. Mines GS #92, but don't remember what date it was built off hand.

It wouldn't surprise me if a couple months (or less) into the build, that too many mistakes were being made during assembly (ie LT1 heads on LT4 or vice versa) that the change was made. Pure speculation on my part.
