Module 1 is CCM and module 4 is ECM.
There is no code 12. 12 is a delimiter intended to show where code sequence starts and ends.
The H is a stored "History" code. The 64 is a right bank o2 sensor circuit code.
Since it's a history code you should clear it by disconnecting the - post at battery for a minute to clear the codes. Then run the car and see if it throws them again.
