problem during hard throttle higher rpms

54K miles
Opti replaced at 10K miles (has not been exposed to moisture since then, garaged).
Engine shudders a bit under hard throttle and higher rpms. Noticeable decrease in power, kinda stumbles/vibrates, between 3000 and 5000rpm. Slightly rough idle (or may be my imagination?).
Do spark plugs typically need replacing at only 54K miles? They've never been replaced, nor have the plug wires. All wires appear tight, no chafing (at least what I can see topside with a flashlight).
