Usually comes on after about 10-15 min. on the highway. Car passed smog and doesn't set other codes.
a. Do I have to worry about this right now?
b. Have others fixed this and what is the most common cause/fix?

Well you can ignore if you want, but if you want to fix it, guess what? You have to take the intake apart, wont that be fun. Once you've done that you can see if the EGR valve is hard to move or is stuck open. In either case replaced it. It could also be the solenoid which is the little black thing on the passenger side of the distributor.
Generally its a hwy only code, as the conditions when it sets only happen at part throttle cruise once the engine is over 170F.
You dont have to fix it immediately, its not a crucial problem.
Generally its because the solenoid/switch has failed, or the EGR pipe is plugged up with carbon, or something with the valve itself.
do a search for this one, lots have discussed it already, it seems it is usually the switch, but it is easy enough to diagnose, do a search and you will find a detailed procedure on how to do it.

I got this code for a little while after I cleaned my throttle body out. I could not figure out why I would all of a sudden get this error after simply cleaning the TB. Turns out when I took the tube off or on going into the intake I knocked off a vacuum hose underneath the intake. It was a tiny little hose but it was all it took to give me that code.
1.there is a switch screwed into the remote egr pipe near the distributor. This switch goes bad, and is easy to change
2.The solenoid is easy to change. Sometimes the clip that holds the egr solenoid electrical connector breaks off, and you have to tywrap the connector onto the solenoid until yoiu get another connector.
3. sometimes a wire backs out of the solenoid connector
4. egr valve gets sticky
5. egr vacuum line on the intake manifold gets plugged up, and you just poke a piece of wire in the hole to clear it, and then the solenoid gets enough vacuum to pull the solenoid.
