The HCs are high and looks like only when at idle or decelerating. They also detectd a HC leak at the right rear of car @39 PPM.
It’s had this problem since I have had it and I took it in to get it diagnosed a few years ago and they said it was the CATS. I changed these and the problem was still there. I have also changed plugs, wires, O2 sensors, and cleaned injectors. The replacement CATS I used were some cheap ones and look undersized so may still be the cause.

I think he said 39 parts-per-million, not RPM. Don't know what he meant about the rear of the car though.
HC is unburned fuel, in the exhaust it is often caused by misfire if the vehicle has no other failures. HC at the right rear could be a gas cap problem or evap system leak, they are designed to contain and burn fuel vapors that would otherwise escape to the atmosphere.
Do you have the HC and CO numbers? It would make it easier to diagnose.
I had a similar problem recently and it was bad gas! HC was 450+ at idle. Drained old gas and refilled with new premium and the number droppted to ~150. 220 was the high pass number.
