You trans has a hurt 3-4 clutch pack, and possibly (although doubtful), and servo/band issue.
A rebuild is required. That is as simple as I know how to say it.
To anticipate the next questions:
1) Changing the fluid and filter will not help.
2) No. A repair is not feasible, as it is internal, and almost full disassembly is required to reach the problem.
3) Yes, you must change the torque converter, as it is worn, and debris from the failure is trapped inside.
4) Depending on where you are in the US, $1000-$2500 is what it normally runs.

Pete is the resident trans expert so his advice is as sound as you will find. The fact that you are shifting 1-2 normal rules out a simple adjustment.
He has answered these questions may times as you can tell.
