Check out your belt tensioner and make sure that it still has the torque needed to keep your belt at the proper tension. There are wear marks on the tensioner which allow you to see if your belt is stretched beyond usefull length. Or, if your tensioner is original or old, you might need to replace it.

This is a good place to start. Things can look good but the indicator marks will tell you if you are geting proper tension.
If you can get the belt off with no effort at all, it may be stretched out and to loose. You normally have to fight with a new belt to get it on and over the last pully.
