All those plus the manual indicates the ignition switch, although I would think you would have other problems if that was the case.
The list is;
Gear Selector Switch misadjusted
Loose Connections
Blown Fuse
Defective Backup Switch
Defective Ignition Switch
Disconnect the backup switch and insert a paper clip, turn-on ignition and see if they light or check the switch terminals with power off and connecting a ohmmeter check for continuity.

I think I have the same problem and have a replacement switch - question is - where the heck is it? Do you get at it from underneath, or can you replace from top by removing the console?
