I had the pushrods pop off the lifters twice on my old engine using roller tips (see pic below of what happened on one occasion),
my theory is that the old style rockers can rock left and right if there is too much clearance combined with some regular rpm, and the pushrod slides out from under it.
Never had a repeat using full roller rockers and magnum pushrods.

..Ed, I have seen a few rockers jump off the valve tip over the years..In every instance it was with self aligning rockers...You may want to replace the lifter in that bore, because as someone else mentioned, it may have collapsed causing the rocker to come off the tip..As far as losing 2 gallons of coolant, if it's not coming out somewhere, or going into the oil pan ,then the only conclusion can be that the engine was low 2 gallons to start with...I would also replace the rocker on that cylinder just to be safe.....WW

I blew up your Last picture in Post #1 and what I found Odd was the Rocker in Question appears to have a lot of "Burnt" Carbon on it's back side, where as the other Rockers look normal.. some how this Rocker was running really Hot..

Also, you do have a leaking Head gasket, your Head looked the same as mine and I was also going through antifreeze.

did you ever resolve this?

i have confirmed i have a leaking head gasket now and am doing some research over the lunch hour on how to fix it. wondering if you recently did yours?

The coolant may not be leaking into the engine at all. One possibility to consider if you are aware of it ... the sides of our stock radiator are made of plastic. After years of heat cycling the plastic develops hairline cracks and fluid just seems to disappear to the point where the radiator has to be replaced.
