Default ECM code 45
'91, L98, 106k miles, auto. The ECM has set a code of 45 (rich exhaust indicated). Does anyone know what is the most probably cause?

I would start with checking the TPS (throttle position sensor) voltage.
A GM manual is necessary.

First place to start would be with a scan tool and check your 02 sensor volts. There a few things that have to happy at at the same time for this code to set. Also check for exhaust leaks. This can trick the 02 sensor and give false readings which lead to codes.
