I see you been working, Egr valve you replace it. Looking at manual, it could be fuel pressure to high, rich injector, lower then normal vacuum can cause it, Disconnecting the MAP will allow the ECM to set a fixed value for thesensor.Substitute different MAP sensor if the rich condition is gone while the sensor is disconnected. You need a "SCAN" Tool to check 02 sensor there outher ways.It could be intermittent. I have the same problem with my 90. No time to work on it yet.

Code 45 is rich exhaust. My $ is on a leaky injector or a sticking one. Your injectors can be getting pulse fine and not be leaking but sticking open sometimes, then closed sometimes intermittently. About the only way to know for sure about sticking is to listen to them with a mechanics stethoscope at idle. You can listen with a piece of rubber tube in a pinch. Listen for a steady ticking with no pause. Sometimes you have to listen to each injector for several minutes each, at least, to catch it. What happens when injector sticks open is it floods the cylinder momentarily causing the miss and the rich exhaust code.

The way to check for leaking injector(s) is to put a pressure gauge on the schrader valve and see if your fuel pressure drops drastically over say a 40 minute period with car not running. If your smelling fuel in your garage after a nights storage then it's likely you have a leaker or two.

My 90 had the exact symptoms you guys are experiencing and the code 45 and I had 2 injectors sticking intermittently. I threw in a set of the new style ACCEL #24's and she's running like a cat on fire.
