What could be causing this error code???

What could be causing this error code???
I have had DTC P0152, which is Circuit High Voltage, Bank 2, Sensor 1 come on 4-5 times now.
I have put in 4 new oxygen sensors & the code has set twice since then, so I doubt if it is a bad sensor....
Seems the code sets maybe about 2-3 miles down the road after a cold/cool engine start......could be related to going closed loop????
This has only happened few times, so it usually does NOT happen..
Could this be indicative of a bad injector or something else?????
