Generally, if you can grasp the tire at the 6 and 12 o'clock positions, with the wheel off the ground, and feel play, there's a strong possibility the bearings are worn. Also, a "growling" sound while rotating the whel, is another sign.
Yes, you buy an entire hub/bearing assembly, the bearings, unfortunately, are not available seperately. My preference for hubs is the SKF brand.

Wheel bearings will generally start to "groan" or protest in some vocal manner when they start to go bad. Most bearings now are sealed and will run for almost "forever". For unsealed bearings like are used in front wheel applications, you will have to remove the calipers and the hubs. You can then clean up the bearing and inspect the rollers and races for pits or other failure. I have never heard of a wheel bearing failure for years and years. They are incredibly reliable. If they are not protesting, they are probably in good operating condition. Don't fix it if it aint broke.
