Both cars were drivable. 58 year old lady was being motioned by a stopped car telling her it was ok to go.
Bummer. Hope you weren't at fault. I've seen countless near misses from these situations. People either assume the person waving them on has checked all other pathways on their behalf, or they get antsy because they're being waited on, or they just plain don't think because it's a fairly uncommon situation that turned into a distraction. I'm super cautious now before giving anyone the go ahead, most of the time I'll just give them space and make eye contact and nod..

We learned in drivers ed to never wave anyone to go ahead. You could be setting yourself up for liability. I am always cautious to take someone elses direction that it's all clear to go.
Never wave a pedestrian to cross. Just sit and wait.

People are becoming worse and worse drivers. Too many distractions, everyone is in a hurry to go nowhere and they are not courteous anymore.
And to make things worse, all the new cars have flat screens to play with - if you are looking at the dash/console, you are not looking at the road. I remember in my 1972 Corvette I got stopped (back in 1973) and was ticked for havi

Gotta love all these people that play traffic cop in their cars. Anytime someone starts waiving at me, I just ignore it - they are not going to pay for the results if they are wrong!

What did you have? For the love of God, man, you can't just leave us all hanging like that!
