P1416 is "AIR System Bank 2". The "h" and "c" means history and current.
Maybe someone can tell you what the code is telling you. All I can say it has something to do with your AIR pump system. (Pollution system).

"The AIR pump is used on this vehicle to lower tail pipe emissions on start up. The PCM supplies a ground to the AIR pump relay, which energizes the AIR pump. The PCM monitors the heated oxygen sensor (Ho2S) voltages in order to diagnose the AIR system....
"If the PCM determines that the HO2S voltages for both banks did not respond as expected during the tests, DTC P0410 sets. If only 1 sensor responded, the PCM sets either a DTC1415 or P1416 to indicate on which bank the AIR system is defective."
Some things to check for yourself - Inspect all pipes and hoses for being connected; secure clamps on the pipes and hoses; no kinks, holes, or pinched hoses/pipes; components with evidence of heat damage.

Steve put you on it well. If you've done any maintenance or mods on it lately, you may have knocked a hose loose or done something else to the system to cause the passenger side AIR supply to become disrupted. The AIR pump itself is in the lower driver-side corner of the front fascia. The supply tube runs from the intake air box where the air filter is. The downstream tube runs from the AIR pump along the inner left front fender in the engine bay, to the driver's exhaust manifold and splits there, runs around the back of the engine to the passenger exhaust manifold. You can see the small tubes that connect through flanges to the manifolds at about the center of the engine on each side. Check all the tubing to make sure there are no leaks and that all the connections are secure.

I had the same code come up on my 98, did some reasearch on anther forum and copied one of the replys from that forum. I cleared the code and have not had the code come back so it has not been a problem for me. I was going to do this maintenance check this winter and so far winter has not come to Okla. my 2cnts
As for the 1416, you probably have a check valve that is NOT working, or rusted shut.
You will have to remove the intake manifold to get to it, but its not bad.
The intake gasket is about $53 and the valve is about $25.
