It was a simple form. Name, address, vin, etc..
They wanted proof of the repair. Receipt or work order.
I was refunded $38.95. The purchase price of the CLB. I tried to bill them for 2 hours of labor at $75 an hour, but they didnt go for it.
If you dont have that form, call Chevrolet Customer Assistance Center at 800-222-1020 and see if they will send out another one
i have had my 98 6speed since july and it has 96k on it, don't no if the other owner ever hade anything done so i'm going to take it in , will this mess up the computor? just asking because i no that my flobs have to be relearnd after i haven't drove it for 3 days , any help would be thankful, just got my recall today
