Default Engine Stumbling...wanting to quit

I posted this also in the tech section...under HELP..engine sounds like it is wanting to die..something like that...anyway here is the problem.

The past few times I have taken my car out it has been doing something very strange that it has never done before. While I am driving usually at a lower RPM or when a decellerate the engine sounds like it is quitting and the car jerks and hesitates and then keeps moving. I have a few ideas of what it could be, some of which are easy fixes and others could be expensive. However, I would like other suggestions/opinions on what the problem might be and how I might trouble shoot the problem. My car is heavily modified and this is the first time I have ever had any trouble with it. Although I have been getting the following code for a long time....

P0410 AIR System, it also has an H and C after it. I still have no idea what it means. I usually just clear it and it does not come back for several weeks but not this time, it keeps coming back.

Any and all input and/or suggestions are welcome.

I m not sure what all codes you might have turned off.
But here is the definition of P0410.
If the PCM determines that the HO2S voltages for both both banks did not respond as expected during the test it sets a P0410 code. The "H" means history and the "C" means current faults. Check to see if codes 412 or 418 are still active in your engine diagnostics of ls1edit. If they are on and not setting check the air pump fuses there are two. Also check in your edit program for air pump disable temp.
I WILL STOP FOR NOW. Let me now what you find then we can continue.

That looks like the code I got immediately after replacing my front crash bar when I forgot to plug in my AIR pump. The AIR pump is directly below the headlamp on the driver's side. There should be two air hoses on the inboard side, and a 3/4 wire electrical connector on the top outboard side. To reach the connector without breaking out any tools, go in from the the front of the hood area on the inboard side of the frame rail, or thru the brake duct opening. It's a tight fit, so I hope you have really skinny arms.

I have a Halltech TRAP which does not accept an AIR tube very easily. Perhaps the tube is crimped/clogged and not working properly?
