A tune will use some type of ECM software to first baseline what is currently happening (air\fuel ratio etc...)then they can modify certain tables to adjust for performace. Bump up the timing some, remove torque management, shift firmness, set the fans lower for a 160* t-stat etc...
I can tell you that the tuner in my area has been able to get 15 to 20 HP out of a stock C5! It helps of course to have mods already done prior to a tune (long tube headers, cam\heads etc...) that way they can adjust for your new found improvements.
