QTP Cut out failures

QTP Cut out failures
Last year the QTP's were acting funny on the C5. I called QTP and they told me to tighten up the set screws on the spindles.
Did that but still something wasn't right.
Yesterday had my spring oil change and while it was on the hoist we checked them out.
One was stuck, but operating at least...the other seemed to butterfly opened and closed freely...then we noticed the motor hanging underneath the car.
The 2 screws that hold the motor had fallen into the cutout itself. We carefully took it all apart, re-assembled it, but no luck, won't even turn, same with the other one now.
Pretty simple systems once we looked at it, anyone had anything that stands out that is simple to fix or check?
I just wanted to get an idea of what to look for, next time its on the hoist someone will be in the car to activate them and check them.
